Case Management Hawaii (CMHI), dba for AAA Kama’aina Health Care LLC, was founded in 2022 with the sole mission of improving case management in Hawai’i for individuals and their families. Our founder is a long-time Case Manager who believes in providing the best possible care for families, and helping care providers care for our seniors.

Our Vision
A healthy, sustainable community where those requiring care are accounted for, well-tended to, and thriving with no one falling through the cracks for lack of finances or support.

Our Purpose
Improve the care and quality of life for everyone we serve whatever their means, whether they are privately paying or Medicaid clients.

Our Mission
Modernize the existing Case Management system to improve client oversight and outcomes; reduce unnecessary emergency care visits and hospitalizations; and provide increased coordination, communication and transparency between client, family, caregiver and providers.
